Consultation Form

BOOKING POLICY

- Deposit is required to book appointment

- $100 deposit for all tattoo sessions

- Deposit is non-refundable and goes towards the cost of your FINAL session

- 48 hours notice required for cancellation, otherwise deposit will be forfeited.

Appointment Preparation Instructions

  • HYDRATE- Drink plenty of water to keep your skin plump, elastic, and ready for ink

    MOISTERIZE- Regularly apply unscented lotion to the area getting tattooed up until the night before to avoid over moisterizing.

    PROTECT YOUR SKIN- Ensure you do not get too much sun exposure or cuts and scrapes to the intended area to be tattooed.

    SKIP TANNING AND EXFOLIATION- Avoid scrubbing the skin right before the appointment to avoid irritation to the skin.

  • EAT A HEAVY MEAL- Have a solid meal within 1-2 hours before your appointment. This keeps your blood sugar stable and significantly helps prevent dizziness or lightheadedness.

    BE CLEAN- Shower before you come and use deodorant. You and I will be close quarters and hygiene is crucial

    DRESS ACCORDINGLY- Wear loose and comfortable clothing that you are not attached to. If any ink gets on your clothes, it will stain.

    DO NOT SHAVE- Allow me to shave the area fresh for the tattoo to avoid accidental cuts or irritation that can cause a reschedule

    NO NO’S- Do not drink the night before or day of. Do not be under the influence of any drugs, and DO NOT use any numbing creams, your appointment will be rescheduled and you will forfeit your deposit and have to pay a new one

  • GOVERNMENT ISSUED ID- This is required to get tattooed legally

    PAYMENT- Bring cash or be prepared to pay with Zelle or Cashapp.

    ENTERTAINMENT- Bring headphones, tablet, etc…. I do have a TV in the booth and can play any music, shows, or movies you would like. You do not need to bring a charger, i have plenty.

    COMFORT ITEMS- Bring a hoodie or blanket if you get cold easy. Your comfort is important.

    A GOOD ATTITUDE- We will be spending several hours together, please be in a friendly and fun mood. I will be
